THIS SWEEPSTAKES IS OFFERED ONLY TO LEGAL RESIDENTS OF THE UNITED STATES (EXCEPT FLORIDA & PUERTO RICO) WHO ARE LOCATED IN THE U.S. (EXCEPT FLORIDA & PUERTO RICO) AT TIME OF ENTRY. NO PURCHASE NECESSARY. A PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. VOID IN FLORIDA, PUERTO RICO, AND WHERE PROHIBITED.
1. No Purchase Necessary. Sweepstakes is sponsored by Bellacor.com, Inc. ("Sponsor"), located at 2425 Enterprise Drive, Suite 900, Mendota Heights, MN 55120 ("Sponsor's Address"). January 1st, 2006, to December 31st 2006, at 11:59 a.m. CST. The "Drawing Date" is January 9, 2007. No purchase or payment is necessary to enter this Contest.
2. How to Enter. Complete and submit an entry form online at www.bellacor.com. Limit one entry per person per day for the duration of the contest. To be eligible, entries for the Contest must be received no later than the Entry Deadline date. All entries become the property of the Sponsor and will not be returned. Sponsor assumes no responsibility for lost, illegible, incomplete, damaged, late or misdirected entries, all of which will be void.
3. Selection of Winners. Eligible entries for the Contest will be entered in a drawing, to be held on the Drawing Date, from which 1 winner will be chosen at random. One (1) Grand Prize winner will receive a gift certificate worth $2,500. The decisions of the judges regarding entries and winners shall be final.
4. Claiming Prizes. Each potential winner will be notified by email or overnight mail and will be required to sign and return an affidavit of eligibility and liability release within ten (10) days of the date of notification, or the prize will be forfeited and an alternate potential winner will be selected. Return of any prize notification as undeliverable will result in disqualification and selection of an alternate potential winner.
5. Eligibility. Sweepstakes is open to individuals, 18 or older and who are legal residents of the United States (except Florida & Puerto Rico) at the time of entry. Employees of Bellacor.com, Inc., and their respective parent companies, subsidiaries, advertising and promotional agencies, and their respective affiliates, distributors, dealers and agents ("Bellacor.com and its agents") and the immediate family members and members living in same household of each are not eligible.
6. Internet. If for any reason the Sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes which corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Bellacor.com, Inc. reserves the right, at its sole discretion, to disqualify any individual who is responsible or who tampers with the entry process, and to cancel, terminate, modify or suspend the Sweepstakes, and select winners from all eligible entries received prior to the termination. Bellacor.com, Inc. and its agents are not responsible for any problems or technical malfunctions of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any email or entry to be received on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof, including any injury or damage to any entrant's or any other person's computer related to or resulting from participation in the Sweepstakes.
7. Winners' List. For a list of winners, send a self-addressed stamped envelope to: Bellacor.com $2,500 Bellacor Sweepstakes. Winners' List Request, 2425 Enterprise Drive, Suite 900, Mendota Heights, MN 55120 to be received by January 19, 2006.
